Submit Records

 Proof of immunity may be provided by:

• Providing a copy of your Illinois School Certificate of Child Health which documents compliance with the Illinois immunization law.  This record is available from your high school.  OR
• Providing a copy of your immunizations from your physician's office.  OR
• Filling out the Southern Illinois University Edwardsville Immunization History Form (must be signed by a licensed health care provider)

Submit electronically via Cougar Care, your secure health portal.

Failure to submit proof of required immunizations by the 1st day of attendance will result in a registration hold for subsequent semesters and a $25 non-compliance late processing fee.
