How to Apply to Upward Bound Math and Science

Applications can be found in the Counseling Office or Room 109 at CHS, or online at www.collinsvilleubms.com
Completed Application Packets Include:
- Two recommendation letters filled out by high school teachers, counselors, and/or administrators.
- Proof of parent/guardian income, health insurance information, and a current report card.
- Parent and student signatures.
Applications can be turned into your school guidance counselor or to the Upward Bound Math and Science offices at the SIUE East St. Louis Center. Once the completed application packet is received, you will be notified by mail or phone to schedule an interview.
Hours of Operation:
All UBMS offices can be reached by phone between 9:00 - 4:00 PM.
During the academic year students meet every 2nd and 4th Saturday from 9:00 - 12:00 PM
Cahokia, East St. Louis & Madison: during the Summer component students reside on SIUE main campus from 7:00 AM Monday (Dropped off at SIUE ESLC) through 1:30 PM Friday afternoon (Picked up from SIUE ESLC). Students are bused to and from the East St. Louis Campus.
Collinsville: during the Summer component students reside on SIUE main campus from 7:00PM Sunday (parents drop off at Bluff Hall) through Friday morning or afternoon (pending field trips). During non-residential programming, students are bused to and from the SIUE main campus.
