You should specify a billing address if you want your bills, refunds, and Tuition
            Payments Statement (1098-T) for Hope and Lifetime Learning Tax Credits to be
            mailed to an address other than your  local or permanent address.  If you are living
            on campus and want your bills to go to your parents or another third party, you must
            specify a billing address.  You can change your billing address on the CougarNet or
            by filling out a Change of Address Form at the Service Center,  at the Bursar's Office
            or you can email us at bursar@siue.edu with the following information:

                                Student Identification Number 
                                Name (last, first and middle)
                                Effective Date of New Address (month/ day/ year)
                                Street
                                City                   State           Zip Code
                                Telephone Number - Please include Area Code
                                Country  ( if outside US )

           If this new address is an  update to your local and/or permanet address,
             please specify and we will forward your new address to the Service Center.
