COVID Self-Reporting Form

You should complete this form if you are aware that you:

  • Have been tested for COVID-19, with or without symptoms
  • Have tested positive for COVID-19 or have been given a diagnosis of presumed positive by a health professional
  • Have symptoms, but not yet tested positive
  • Have had a CLOSE contact with an individual who has been tested and diagnosed with COVID-19

In alignment with the Centers for Disease Control's definition of CLOSE contact, reporting is necessary if you are someone who was within 6 feet of an infected person for a cumulative total of 15 minutes or more over a 24-hour period starting from 2 days before illness onset (or, for asymptomatic patients, 2 days prior to test specimen collection) until the time the patient is isolated.

Your identity will not be disclosed to anyone other than SIUE employees responsible for maintaining your health and safety, or Health Department officials as required. You may voluntarily share your information with whomever you choose.

Winter Break Information

Cougar Village will begin Winter Break operations on December 18th at 5pm. We will resume regular operations for the Spring semester on January 15th at 10am.

Twenty-four-hour quiet hours start at 5:00 pm Friday, December 11, 2020, and continue until noon Saturday, December 19, 2020. During this time, NO NOISE should be heard coming from any room, apartment, or common area at any time, day or night. Please be respectful of your neighbors during this stressful time!

Members of the Residence Life staff will enter your apartment on Friday, December 18, 2020, to complete safety and security checks to ensure living units are safe and secure during Winter break.

All residents should take safety precautions, especially if your apartment were unoccupied during any part of the break period. Break periods are prime times for thefts and burglaries, but taking the following safety precautions can help keep your apartment protected.

As a reminder, if you have an open space in your room (your roommate left or is leaving), you need to make sure half of the room is clean and ready for a new roommate to move in before leaving for the break!

Before you leave, don’t forget to:

  • Deadbolt your door
  • Lock all windows
  • Lock sliding doors with latches and security sticks.
  • Bring your bike inside. Locking it up outside will leave it susceptible to theft.
  • Please make sure to remove all perishable items in your refrigerator before your departure for the break.
  • DO NOT GIVE YOUR KEY TO ANYONE.
  • Close all curtains
  • When possible, take computers, TV/stereo, and other valuables with you
  • Utilize a light timer to turn lights on and off

To conserve energy during the break period, we also ask that you:

  • Set the thermostat to the lowest setting (~68 degrees), but DO NOT TURN YOUR THERMOSTAT COMPLETELY OFF!
  • Unplug unnecessary appliances (i.e., hairdryers, curling irons, blenders, toasters), but DO NOT UNPLUG THE REFRIGERATOR OR STOVE.
  • Check the stove and oven to make sure that they are both off.
  • Turn off exhaust fans because these can catch fire!!

As a reminder, UHFM may enter rooms during the break to complete any work orders and to complete preventative maintenance.

Pest control will continue to service over break without exception. REMEMBER TO REMOVE ALL GARBAGE AND RECYCLABLE MATERIALS BEFORE YOU LEAVE TO PREVENT PESTS.

Health and Safety Tips

Sign up for e-Lert today!

What is e-Lert?

SIUE's e-Lert is a text messaging service that will be used to quickly notify faculty, staff, and student cell phones when an urgent situation, such as a bomb threat, severe weather, hostile intruder, or when the University closes due to inclement weather. There is no cost to register for the alerts; however, standard rates may apply if your cell phone plan does not include text messaging.

How do I register for SIUE e-Lert?

If you are a student, faculty, or staff member, you can register by clicking the "Sign Up Now" link and entering your active e-id, password, and cell phone number. Each time you are notified to change your e-id password, you will also be given an opportunity to enroll, update your cell phone number if it has changed, or cancel your registration with the system.

You will still receive alerts as long as you have the same cell phone number.

Rave Guardian

Southern Illinois University Edwardsville (SIUE) now offers the Rave Guardian app for its students and their families. Rave Guardian is one of the best ways to not only improve personal safety but also protect your friends by building a private safety network. This app provides instant communication with friends, family, campus safety, and even 9-1-1.

SIUE encourages all students throughout campus to utilize the app to network with friends and help increase the overall safety and security of the entire student body. An important part of the Guardian app is that students can use it not only to keep themselves safe but to help protect others. Even students who may not feel as if they need others to help keep them safe can utilize the app to be a guardian and help protect their friends.

Download Rave Guardian and Become a Guardian on Campus
